<p>Dr. Osterholm is director of the Center for Infectious Disease Research and Policy (CIDRAP), associate director of the Department of Homeland Security&#8217;s National Center for Food Protection and Defense (NCFPD), and professor in the School of Public Health, University of Minnesota. He is also a member of the Institute of Medicine (IOM) of the National Academy of Sciences. In June 2005 Dr. Osterholm was appointed by Michael Leavitt, Secretary of the Department of Health and Human Services (HHS), to the newly established National Science Advisory Board on Biosecurity.</p>
<p>&#13;</p>
<p>It is estimated that only 18% of all U.S. companies have any plans in place to meet the issues of an outbreak of the Avian Flu as it relates to a human pandemic influenza or fear of a pandemic. If the Avian Flu gets to the U.S. it may well have only a minor impact to the stakeholders.  If however, it mutates into a human pandemic influenza, the economy could easily become paralyzed. People would isolate themselves in their homes, thus stop going to work, school and public venues. Unemployment insurance and government support services would be difficult if not impossible to access. As time continued banking, food supplies, the Internet and phone services would become disrupted.  It is estimated a pandemic could cause up to 30% of all small businesses to go into bankruptcy.  Referring back to the nightmare results of Katrina, as an example, it is easy to predict that the number could easily go higher.</p>
<p>&#13;</p>
<p>An AI outbreak in the U.S. will have little impact on most individuals. In contrast, however, a human epidemic in the U.S. or even overseas would impact a greater number and create fear and disruptions on a global basis.  A pandemic could be biblical in global devastation.  It would impact the U.S. military, police and other security aspects of daily life.  Few if anyone would not be touched by death and sickness.</p>
<p>&#13;</p>
<p>·    57% of Americans reported concern about the spread of Avian Flu.</p>
<p>&#13;</p>
<p>·    15% said they were very concerned</p>
<p>&#13;</p>
<p>·    20% believe they or a relative will get the Avian Flu.</p>
<p>&#13;</p>
<p>·    75% said they would avoid travel.</p>
<p>&#13;</p>
<p>·    71% said they would avoid public events</p>
<p>&#13;</p>
<p>Current government recommendations for families and business suggest the stockpile of critical supplies before a human pandemic occurs. It is estimated that these supplies should last from 3 weeks to 3 months and depend on the impact to the logistics of a community. Companies should create work at home plans and provide key employees with “Flu Kits” as preparation for the contingency that the virus migrates into a human transmittable strain. A pandemic event could last 12 to 18 months.</p>
<p>&#13;</p>
<p>At the time of this writing, no human AI-related pandemic virus has been identified.  The U.S. food supply remains safe and world health agencies are working to control this aggressive bird-related disease that has been in existence for centuries if not longer. But that could all change if this virus mutates. And many elite scientists believe it&#8217;s not just a matter of if, but when.</p>

<p> A gene related to rapid replication and lethality has been found to be present in, and to contain some identical protein sequences in, tobacco mosaic virus and non-small cell lung cancer. The early detection diagnostic test for cancer, the AMAS® test, appears to measure this gene&#8217;s activity. </p>
<p>&#13;</p>
<p>This is the first of a series of reports on the use of a new methodology to isolate &#8216;in silico&#8217; and track specific gene activity in normal and several disease states. </p>
<p>&#13;</p>
<p>This newly identified gene has been named the Replikin Peak Gene (RPG). &#8220;Replikins&#8221;TM are a class of proteins containing high concentrations of amino acids lysine and histidine that have been previously determined to be related to rapid replication and virulence. Using proprietary computer algorithms constructed to identify, count and track historically this class of proteins, replikins have now been analyzed in 130,488 protein and genome sequences. These include all the accession numbers for common strains of influenza and other lethal virus isolations published between 1917 and 2007 in the PubMed repository. Replikins have been found to be concentrated in specific genomic areas. The area of each genome with the highest concentration of continuous replikin peptides has been isolated in silico and named a Replikin Peak Gene (RPG). </p>
<p>&#13;</p>
<p>Specific RPGs have been identified and quantified in infectious organisms including viruses, bacteria and trypanosomes (malaria). &#8216;Upregulation&#8217; of the Replikin Peak Gene is evidenced by an increase in the replikin count in the RPG, that is, in the number of replikins per 100 amino acids.  For example, in the case of influenza B, which has not to date been lethal, a low replikin count (less than 4) always occurred in influenza B RPGs between 1940 and 2006; and in quiescent periods of influenza A. In contrast, the replikin counts in lethal influenza A virus RPGs increased to over 20, from as much as one to three years before and during outbreaks. Strain-specific increases in replikin count have appeared in relation to the major influenza pandemics and epidemics, after years of constant low stable counts. Furthermore, a high replikin count of the RPG of an organism has been shown to be associated consistently with a higher percent lethality in the host. The increase in count was frequently detected one year or more before the outbreak was clinically apparent. The ability to identify, count and track replikins in the genomic structure of an emerging organism has been shown to be accurately predictive in advance warning of H5N1 outbreaks, and may have value in the management of influenza epidemics.</p>
<p>&#13;</p>
<p>The highest replikin count of the RPG found in any organism studied to date is in lethal non-small cell lung cancer. The highest AMAS test results found to date also are in lung cancer. Tobacco mosaic virus has some sequences in its RPG that are identical to those in the RPG in non-small cell carcinoma of the lung. Two structural studies have suggested mobility and inter-organism transfer of replikins associated with lethality.</p>
<p>&#13;</p>
<p>Replikins have been chemically synthesized and were found experimentally to be immunostimulants, producing strong antibody responses in animals. Synthetic cancer replikins administered to animals produce antibodies that can be measured by the AMAS cancer test.  The AMAS test&#8217;s results are evidently related to the rate of replication in the cancer.  In short, the AMAS test measures the activity of the replikins that are unique to cancers, whose concentration is related to lethality.</p>
<p>&#13;</p>
<p>High replikin counts are present in a wide range of lethal diseases. The structural and functional relationship found between the replikin peak genes of the tobacco mosaic virus and non-small cell lung cancer is one illustration of the relationship between replikin count, rapid replication and virulence. Possible implications of these findings of the same replikin structures and functions in tobacco mosaic virus and lung cancer are under further study.</p>
<p>&#13;</p>
<p>Detailed data on these studies are the subject of issued and pending published patents and are being presented today at a seminar in New York by Samuel Bogoch, M.D., Ph.D.   </p>

<p> The City of Laredo Health Department (CLHD) and the Laredo Independent School District (LISD) are installing a &#8220;first-in-the nation,&#8221; Internet-based system for early identification of possible novel Influenza H1N1 virus infection of students, teachers and staff. The virus is commonly known as the &#8220;swine flu.&#8221;</p>
<p>&#13;</p>
<p>The system, known as Argus1, also provides decision support for school nurses and other providers in caring for sick students; instant alerts such as vaccine recommendations, and continuous reporting and tracking of outbreaks to the CLHD and school authorities. </p>
<p>&#13;</p>
<p>&#8220;In addition to Influenza A H1N1, the system also provides point-of-contact surveillance for other forms of influenza as well as the potential to track other public health infectious diseases and chemical agents that the U.S. Dept. of Homeland Security has identified as possible agents in a biological or chemical terrorist attack,&#8221; said Robert Burns, Director of Information Technology for Argus One Systems, Inc.</p>
<p>&#13;</p>
<p>Argus1 was introduced in Laredo as part of a test site from 2003 to 2006. Its re-deployment for the 2009-10 school year is based on extensive testing and refinement of the system, with particular attention to the new Influenza A H1N1 virus. Argus1 also can be used by physician offices, emergency medical responders and hospital emergency departments, Burns noted.</p>
<p>&#13;</p>
<p>Patricia Keck, Director of LISD Health Services, stated:</p>
<p>&#13;</p>
<p>&#8220;Argus 1 will be an important part of our &#8216;Fight the Flu&#8217; program. By quickly receiving reports from school nurses, LISD Health Services Department in conjunction with the City of Laredo is able to track illness patterns. If an illness pattern develops at an LISD school, we will provide the school with intensified flu prevention recommendations.  Argus1 is a valuable weapon in fighting flu outbreaks.&#8221;</p>
<p>&#13;</p>
<p>&#8220;In addition to assisting nurses caring for sick students, Argus1 provides a distinctive reporting function to the CLHD, which has played a key role in the design and development of the system during the past six years,&#8221; said Vincent Friedewald, MD, Medical Director of Argus One Systems.</p>
<p>&#13;</p>
<p>&#8220;This surveillance system is an important adjunct to track and monitor disease from the onset at the provider level and to quickly report to the CLHD for a rapid response intervention to contain disease,&#8221; said Hector Gonzalez, MD, MPH, Director of the city&#8217;s Health Dept. </p>

<p>&#8220;The system is secured and uses both landline and wireless Internet-connected computers,&#8221; explained Friedewald, a Clinical Professor of Medicine at the University of Texas Health Science Center &#8211; Houston. </p>

<p>Argus1 assesses signs, symptoms, test results</p>
<p>&#13;</p>
<p>&#8220;An important part of the program involves assessment of patients&#8217; signs, symptoms and test results, which are instantly transformed into a comprehensive list of possible disease causes and detailed information about each of the diseases,&#8221; he added.</p>
<p>&#13;</p>
<p>Additionally, when a person&#8217;s symptoms match any of nine categories of public health syndromes, including influenza-like illnesses, the exact time and location is reported to both the CLHD and to the central office of the School District, he said. All reported cases are instantly displayed on a map of the City of Laredo so that potential cases are immediately tracked to help officials decide how to prevent its spread throughout the schools and the community at large.</p>
<p>&#13;</p>
<p>&#8220;As the school year begins, we are entering a potentially very dangerous period with a new influenza virus that particularly affects young persons,&#8221; emphasized Friedewald, who is an Associate Editor for The American Journal of Cardiology, as well as a research professor in the College of Science at the University of Notre Dame</p>
<p>&#13;</p>

<p> An innovative new water treatment system could provide the UK’s poultry farmers with an extra line of defence against the spread of avian influenza in the national flock.</p>
<p>&#13;</p>
<p>The Aqualution system, developed by UK company Forum Bioscience, was originally developed to help producers reduce the incidence of food poisoning bacteria such as Campylobacter and Salmonella.  But during regulatory tests required under the EU’s Biocidal Products Directive, the system demonstrated its ability to destroy the avian influenza virus where it’s present in drinking water – believed to be a possible means of transmission between birds.</p>
<p>&#13;</p>
<p>“The Food Standards Agency (FSA) says Campylobacter is the single biggest cause of food-borne illness in the United Kingdom,” explains Tom Ritchie, business manager with Forum Bioscience, “so Aqualution satisfied the demand from producers for a product which could be used successfully during production.</p>
<p>&#13;</p>
<p>“But the speed with which avian influenza has moved across Asia and into Europe has forced producers to consider contingency plans for their flocks, should the disease become commonplace among birds in the UK,” he points out.</p>
<p>&#13;</p>
<p>“Several producers have expressed an interest in installing Aqualution as their extra line of defence.  It provides them with an alternative to conventional chemicals such as chlorine or ozone, where the concentrations required can often have a harmful effect on the birds.  Such treatments are also unpopular with consumers who, understandably, are becoming more ‘savvy’ about the methods employed to produce their food.</p>
<p>&#13;</p>
<p>“Aqualution relies on electrolysis to generate ‘activated water’, containing a very small amount of chlorine (3-5ppm) and high concentrations of dissolved oxygen.  The combination is terrifically effective against pathogens,” he stresses.</p>
<p>&#13;</p>
<p>On a typical unit, drinking water will often contain pathogen counts of 100,000/ml.  Once Aqualution has been installed, this reduces to 10/ml and often zero. </p>
<p>&#13;</p>
<p>“Because the water stays ‘active’ up to the point of drinking, the system helps prevent spread of disease between birds,” Mr Ritchie explains.  “In addition, we have fogging trials underway to look at the effectiveness of the system for controlling airborne transmissions.  If successful, this could then be used inside sheds and control rooms, as well as at farm entrances for disinfecting vehicles.”</p>
<p>&#13;</p>
<p>Independent testing has shown that the treated water satisfies the British Disinfectant Standard against a range of problem pathogens including Pseudomonas, E.coli and Campylobacter, as well as avian influenza and a number of other viruses.</p>
<p>&#13;</p>
<p>The system can also boost end profits and animal welfare.  “Extensive trial results show that for meat production, a combination of reduced bird mortality, increased growth rates and better food conversion are achieved.  In layers and breeders there is also greater egg and chick yield, and litter is drier – easing consumer concerns about ‘hock burn’.”</p>

<p> Replikins, Ltd. has found that the Replikin Count™ of the H1N1 strain of influenza virus has recently increased to 7.6 (plus/minus 1.4), its highest level since the 1918 H1N1 pandemic (p value less than 0.001). A rising Replikin Count of a particular influenza strain, indicating rapid replication of the virus, is an early warning which has been followed consistently by an outbreak of the specific strain. The current increase appears to be specific to H1N1; there was a concurrent 80% decline in the Replikin Count of H3N2, for instance.</p>
<p>&#13;</p>
<p>The current H1N1 appears to be rapidly replicating simultaneously in the U.S. and Austria. It may succeed H5N1 as the leading candidate for the next expected overdue pandemic. However, the same virus replikin structures detected by FluForecast® software in all three previous pandemics, namely 1918 H1N1, 1957 H2N2, and 1968 H3N2, as well as in H5N1, have not yet been detected in the currently evolving  H1N1.</p>
<p>&#13;</p>
<p>There is evidence that many factors, including virus structure, host receptivity, and the environment, together with infectivity and rapid replication, need to converge for a pandemic to occur. For H5N1, the high human mortality rate, which peaked at over 80% in 2006-07 in Indonesia, as well as current low infectivity, both appear to limit H5N1&#8242;s ability to produce a pandemic.  Furthermore, the H5N1 rapid replication cycle which began in 1996 now appears to be over.  The H5N1 virus produced less than 300 World Health Organization confirmed deaths over the past 10 years. </p>
<p>&#13;</p>
<p>On the other hand, H1N1, with an estimated human mortality rate of only 2.5 to 10%, but with much higher infectivity, produced an estimated 50 million deaths in the 1918 pandemic.  A number of countermeasures exist today which did not exist in 1918, however. Among these is Replikins&#8217; ability to manufacture synthetic vaccines based on current sequences, with a seven day production turnaround.</p>
